Form CA-1 (online at ECOMP https://www.ecomp.dol.gov/) This is probably the best way to file (even better with the help of the branch OWCP Specialist). Instructions here. It is probably best not to use a computer at the Post Office.
Form CA-1 (fillable) Federal Notice of Traumatic Injury and Claim for Continuation of Pay/Compensation
Form CA-2 (fillable) or at ECOMP Notice of Occupational Disease and Claim for Compensation.
Form CA-2a Notice of Recurrence (please contact the branch or Bob Jewell for help with this).
